Some States require you to fill out certain forms when holding a fundraiser for your organization. Please find your state below to obtain the necessary fundraiser form. Please remember fundraiser forms must be submitted at the time orders are placed.

Arizona          Fundraiser Form Arizona

Arkansas        Fundraiser Form Arkansas

Alabama         Must have ID number on your letterhead.

California        Schools are not exempt.

Colorado         Fundraiser Form Colorado

Connecticut     Fundraiser Form Connecticut

Delaware        No sales tax

Florida             Fundraiser Form Florida

Georgia           GA issues exempt certificates

Idaho               Fundraiser Form Idaho

Iowa                Fundraiser Form Iowa

Indiana             Fundraiser Form Indiana

Illinois              The exempt organization must apply to the state for exempt status

Kentucky          Fundraiser Form Kentucky

Kansas              The state issues an official certificate of exemption

Louisiana           A copy of the signed and approved R-1048 will serve as the exemption certificate.

Maine                ME requires a letter from state of Maine stating the organization's tax exempt status

Massachusetts    Fundraiser Form Massachusetts

Maryland            Schools are not exempt

Michigan             Fundraiser Form Michigan

Minnesota           Fundraiser Form Minnesota

Missouri              Fundraiser Form Missouri

Mississippi          A letter on your letterhead stating your wish for exemption with EIN number/ FIN number

Montana             No sales tax

Nevada                   The State of Nevada's Department of Taxation issues a letter for tax exemption.

New Hampshire       No Sales Tax

New Jersey             We need a letter on your letterhead stating your wish to be exempt

New Mexico            NM uses "NTTC" forms-to apply Fundraiser Form New Mexico

New York               Schools are only required to supply us with its official purchase order

Ohio                        Fundraiser Form Ohio

Oklahoma                You will have a letter from the state of OK or an actual sales tax exemption certificate issued by the state

Oregon                    No Sales Tax

Rhode Island            The State issues a certificate with a number. Payment for products purchased must come from the organization

South Carolina          SC requires ST-9 Exemption Certificate

**NATIVE AMERICAN INDIANS Native American Indians qualify for tax exempt status on orders placed in their name and shipped to the reservation. We are required by the state to have proof that the ship to address is on the reservation. Depending on the states requirements the following may be acceptable:
  • Letter from the Tribal Council
  • Copy of Tribal Council card
  • State Native American Indian Exemption Certificate (where applicable)
